메타프론티어 맘퀴스트 생산성 지수를 이용한 상급종합병원과 대형종합병원의 생산성 변화 비교
A Comparison of Productivity Changes for Advanced General Hospitals and Large General Hospitals of Korea Using Metafrontier Malmquist Productivity Index

본 연구는 2007년부터 2011년까지 5년 간 대한병원협회의 수련병원 경영실적자료를 이용하여 상급종합병원그룹과 대형종합병원그룹의 확률변경생산함수와 메타확률변경함수를 추정하여 그룹기술효율성과 메타효율성, 메타프론티어 맘퀴스트 생산성 지수를 측정하였으며 상급종합병원과 종합병원 간 기술격차, 규모수익, 그리고 생산성 변화를 비교분석하였다. 본 연구의 분석결과와 시사점을 정리하면 다음과 같다. 본 연구의 결과에서 상급종합병원은 대형종합병원에 비해 기술효율성과 기술격차는 선도적인 입장에 있으나 규모의 비효율성이 대형종합병원에 상대적으로 높으며 규모의 비효율성이 증가하고 있다. 상급종합병원은 규모의 비효율성이 증가함에도 불구하고 상급종합병원들은 지속적인 분원신축과 병상증설이 이루어지고 있다. 이로 인해 종합병원 간 과당 경쟁으로 의료시장의 왜곡이 더욱 심화되고 있다. 따라서 의료기관의 종별 기능을 제도화하고 상급종합병원의 병상증설과 병원신축을 억제함으로써 의료기관 종별로 환자분산을 통하여 균형 있는 병원산업으로 발전하여야 할 것이다
Abstract
This study measured group technical efficiencies, meta technical efficiencies, and meta productivity indices with stochastic frontier production function and meta stochastic frontier functions for advanced general hospitals and large general hospitals of Korea using annual data from 2007 to 2011 of management performance collected by Korean Hospital Association and then analyzed technology gap ratio, returns to scale, and productivity changes. We found that advanced general hospitals were superior to large general hospitals in terms of technical efficiencies and technology gap ratios but their scale inefficiencies were increasing sharply. In the spite of the rising scale inefficiencies, advanced general hospitals built branches and increased hospital beds. The resulting overcompetition among advanced general and general hospitals distorted domestic medical service market further. For a balanced growth of Korean hospital industry with inducing patients to diversely consume general hospitals in terms of function and region, a policy of reorganization of general hospitals and constrain of both the construction of new general hospitals and the expansion of existing ones was necessary.
