병원산업의 수직화와 분리에 대한 소고
A consideration of an integrated vs a disintegrated structure in the hospital industry

병원 산업에 있어 경영 효율성 문제는 학자들 사이에 오래 동안 인식되어왔지만, 병원이라는 특수성 때문에 경영 효율성이나 비용에 대한 접근이나 분석이 미미하였다. 특히 우리나라는 대부분의 병원이 자유 개업 의료인에 의해 직접 운영되는 소형 병원이 주류를 이루고 있다. 따라서 경영의 효율성이나 비용/투자에 대한 개념은 대형화된 병원에서나 겨우 언급되어온 실정이다. 하지만 1980년대 이후 경제적 성장과 더불어 의료 서비스에 대한 수요와 공급 및 개원의 급증은 경영상의 문제점을 드러내었고, 그 결과 정부가 의료 시장에 개입하는 동기를 부여하였다. 이것이 바로 국내의 폐쇄형 병원 구조로 부터 분리된 개방형 병원 구조로의 전환이다. 하지만 이러한 개방형 병원구조를 언급하면서도 막연한 경제성에 대한 설명만 언급되었을 뿐, 구체적인 이론과 더불어 논의되지는 않았다. 따라서 본고는 현재 처하고 있는 우리의 병원 산업의 문제점을 최소 효율성 규모(minimum efficiency scale: MES)와 거래비용이론(transaction costs theory)을 기반으로 개방 병원과 폐쇄 병원체제의 경제적 의의를 산업경제학적 측면에서 비교 분석한다. 즉 수직화 되어 있는 폐쇄형 병원 구조로부터 얻어질 수 있는 거래 비용의 경제적 이점이 최소 효율성 규모를 만족 시키지 못한다면, 역으로 분리화 되어 있는 개방형 병원 구조의 존립 이유가 된다는 것을 이론적으로 논의하는데 목적을 두고 있다. 따라서 개방형 병원 구조가 폐쇄형 병원 구조보다 우위에 있다는 것을 주장하는 것이 아니라, 두 체제가 서로 공존할 수 있는 관계(trade-off relationship)임을 이론적으로 설명하고자 한다.
Abstract
It was the middle of 1990s that the managerial efficiency problem come up to the surface. The hospital industry has grown fast with sustainable economic growth since late 1980, but its financial health has not moved the same way because over-supply of medical service and structural problems erode margins, especially in small and medium size hospitals. This led the government to intervene the hospital market: transferring an open hospital system(a disintegrated structure) from a closed hospital system(an integrated structure) is introduced a way to solve these problems. Two main differences between the open system and the closed system are 1) whether or not physicians co-work with the full-service hospitals and 2) whether or not the organizational structure is highly integrated in services and equipments. Each system has its own merits and demerits: one’s merits are another’s demerits, vice versa.Nevertheless, no explainable theories for the open system and the closed system can be found. This is the reason that we try to compare one to another using two theories, transaction cost economies and minimum efficiency scale. Especially we focus on why these two systems co-exist together rather than which system is better.
