In this study, operational performance of local public medical centers are examined, and efficient management methods are suggested. Based on the real data of local public medical centers from 2003 to 2005, performance development is explored using Malmquist analyses. First, annual productivity indices are explored, and factors affecting productivity are differentiated whether they are changes in efficiency or in technology. Second, contracting-out and direct management method are verified in terms of higher productivity. As a result of analyses, the number of local public medical centers with high productivity index reduced over time. Direct management method improved productivity through internal effort including a reduction of staff and cost containment. On the contrary, contracting-out showed an improvement in profitability with government subsidy and contribution held constant, but achieved deteriorated performance in terms of publicness. From a productivity perspective overall, contracting-out showed a higher productivity index in 2004 followed by direct management method in 2005. In conclusion, contracting-out is not the absolute answer for high productivity in local public medical centers. With this taken into account, local public medical centers must accurately re-establish their functions and roles as public healthcare organizations and secure publicness and profitability.
