소득세의 의료비공제 개선방안에 관한 연구

초록
본 연구는 외국의 의료비공제 제도들을 검토하였고, 의료비공제와 관련된 실태와 현황들을 의료비 공급자와 수요자 측면에서 분석하였다. 분석결과에 따르면 2010년 세무조사에서 의료업종의 소득 탈루율은 28.2%로 높게 나타났다. 또한, 의료비 수요자는 대부분 일반 근로소득자로 2009년 기준 평균 연소득은 25.3백만원 수준이지만, 의사, 한의사 및 치과의사의 평균 연소득은 88백만원 수준으로 추정되어 일반 근로소득자의 3배 이상 수준이다. 소득세에서 2009년 기준 의료비공제 대상자는 217만명, 의료비공제 금액은 4.4조원, 의료비공제 금액을 포함한 총 의료비는 총 급여의 4.4% 수준이며, 최근 총 의료비 비율은 증가하는 추세이다. 본 연구는 이러한 분석 결과를 기초로 의료비공제 제도의 개선방안을 다음과 같이 제시하였다. 먼저, 총 급여의 3%로 규정하고 있는 기준소득금액은 폐지되고 일반 의료비에 대한 한도 규정도 삭제하여, 지출된 의료비 전액을 소득에서 공제하여야 한다. 둘째, 실무에서 의료비 구분이 점차로 모호해지는 복잡한 현실을 반영하여 의료비공제 대상의 범위를 대폭 확대하여야 한다. 마지막으로 실무를 반영하여 사업자의 의료비 공제요건을 단순화하고 성실사업자의 범위를 확대하여, 자본소득 사업자를 제외한 사업자 대부분이 의료비공제를 받을 수 있도록 관련 규정을 개정하여야 할 것이다. 이러한 의료비공제 제도의 개선을 통하여 국민 대부분이 의료복지를 향유하고 삶의 질을 향상시키며, 앞으로 보건복지와 고령화 의료문제가 개선되기를 기대한다.
Abstract
This study is reviewed the system for income deduction of medical expenses, and is analyzed the real state or the current situation in several respects of supplier and demander for medical expenses. The results of this study is proposed the improvements of income deduction for medical expenses as follows;First, the amount of criteria income provided 3% for taxable wage and salary must be abolished and the maximum limit on general expense for medicine must be removed. According to the basis of these results, the total amounts in spending medical expenses must be deducted from income. Second, the range of items regarding income deduction for medical expenses must be drastically extended as the clear lines of demarcation for medical expenses are increasingly fainted in practice. Finally, the individual business conditions regarding income deduction for medical expenses must be simplified and the range of honest individual business must be enlarged, and thus the most of individual business excepting capital income must be favored with income deduction for medical expenses. Through the results of this study the most of people have enjoyed the welfare of medicine and have enhanced the quality of life, and we will look forward to achieve clear and healthy society in the future.
