라섹과 M-라섹 후 각막내피세포의 변화
Corneal Endothelial Cell Changes after LASEK and M-LASEK

목적: 근시 환자를 대상으로 라섹수술시 0.02% 마이토마이신 C (Mitomycin; MMC)를 사용한 군(M-라섹군)과 사용하지 않은 군(라섹군)에서 수술 전후 각막내피세포의 변화를 비교해보고자 하였다. 대상과 방법: 라섹군 57명 104안, M-라섹군 48명 86안에서 술 전과 술 후 3개월, 12개월에 각막내피세포를 검사하였다. 결과: 라섹군과 M-라섹군 두 군간의 술 전, 술 후 3개월, 12개월 경과 후 각막내피세포밀도(CD), 세포면적변이계수(CV), 육각형세포비율(6A)은 통계적으로 유의한 차이는 없었다(p˃0.05). 라섹군에서 술 전, 술 후 3개월, 12개월째 수치를 비교하였을 때 각막내피세포밀도는 통계적으로 유의한 차이는 없었고(p˃0.05), 세포면적변이계수와 육각형세포비율은 술 전과 12개월 수치를 비교하였을 때는 통계적으로 유의한 차이가 있었다(각각 p=0.001, p=0.034). M-라섹군에서 각막내피세포밀도는 술 전과 술 후 3개월째 수치를 비교하였을때는 통계적으로 유의하게 2.8%가 감소하였으나(p=0.004) 술 전과 술 후 12개월째 수치를 비교하였을 때는 통계적으로 유의한 차이가없었고(p˃0.05), 세포면적변이계수와 육각형세포비율은 술 전과 술 후 3개월, 12개월째 수치를 비교하였을 때 통계적으로 유의한 차이는 없었다(p˃0.05). 결론: 라섹군과는 달리 M-라섹군은 술 후 3개월 단기간 결과에서 각막내피세포밀도가 통계적으로 유의한 감소를 보였지만, 두 군 모두술 후 12개월간의 장기간 관찰에서는 술 전후 각막내피세포밀도의 유의한 차이를 보이지 않았다.
Abstract
Purpose: In this study we evaluated the changes in the corneal endothelial cells before and after the operation among myopes in the M-LASEK group, on whom 0.02% mitomycin C (MMC) was used and in the LASEK group, on whom MMC was not used. Methods: The corneal endothelial cell analysis was performed in 104 eyes of 57 subjects in the LASEK group and in 86eyes of 48 subjects in the M-LASEK group before the operation, and 3 months and 12 months postoperatively. Results: There were no statistically significant differences in the corneal endothelial cell density (CD), the cell area coefficient of variance (CV), and hexagonal cell rate (6A) between the 2 groups before the operation, and 3 months and 12months postoperatively (p > 0.05). In the LASEK group, there were no statistically significant differences (p > 0.05) in CD when the numerical values before the operation and 3 months and 12 months after the operation were compared, but there were statistically significant differences in CV and 6A when comparing before the operation and 12 months postoperatively (p = 0.001, p = 0.034, respectively). In the M- LASEK group, there was a 2.8% statistically significant decrease (p = 0.004)in CD when the numerical values before the operation and 3 months after the operation were compared, but there were no statistically significant difference (p > 0.05) when the numerical values before the operation and 12 months after the operation were compared. In addition, there were no statistically significant differences (p > 0.05) in CV and 6A when the numerical values before the operation and 3 months and 12 months after the operation were compared. Conclusions: M-LASEK, contrary to LASEK, showed statistically significant differences in CD in short-term results such as 3 months postoperatively, but in long-term observation such as 12 months postoperatively, both groups showed no statistically significant differences.
