저소득층 유방암환자의 투병경험에 대한 근거이론적 접근-이혼한 한부모 여성가장을 중심으로

본 연구는 근거이론 연구방법을 활용하여 이혼으로 한부모 여성가장이 된 저소득층 유방암환자의 투병 경험을 살펴보았다. 6명의 참여자를 대상으로 심층 면담을 실시하고 지속적 비교분석 방법으로 분석하였다. 연구결과 117개의 개념과 30개의 하위범주, 13개의 범주가 도출되었다. 축코딩에서 패러다임에 의한 범주 분석 결과 인과적 조건은 ‘생활고와 책임감으로 짓눌린 고단한 삶’, ‘예상치 못한 암 선고’, ‘암의 불안과 혼돈 속에 들어섬’이었고, 중심현상은 ‘홀로 감당해야 하는 벼랑 끝 삶에 암의 고통으로 무너져 내림’이었다. 맥락적 조건은 ‘암 치료와 관리에 걸림돌이 되는 가난의 혹독함’, ‘사무치는 고독’이었다. 현상을 조절하기 위한 작용/상호작용 전략은 ‘지켜내야 할 자녀를 바라보며 버텨내기’, ‘삶을 긍정적 자세로 대하기’, ‘홀로 삭히며 현실을 회피하기’이었다. 중재적 조건은 ‘신앙의 힘’, ‘사회적 지지’로 나타났으며 결과는 ‘암을 통해 성장한 나를 발견함’, ‘비참한 삶에 절망함’으로 나타났다. 선택코딩으로 핵심범주는 ‘책임을 떠안은 채 생사의 살얼음판 위에 서서 고통의 칼바람을 헤쳐 나아가기’로 나타났다. 과정분석을 통해 연구 참여자의 투병경험은 ‘진단단계-혼돈과 고통의 단계-대처단계-희망/절망단계’로 나타났다. 연구결과를 토대로 유방암 조기 검진 서비스 제공 및 홍보와 교육이 강화, 비급여 검사 및 치료비를 경감하는 제도적 장치 마련 및 경제적 지원이 강화될 필요성과 가사도우미서비스, 자녀 돌봄 서비스 제공, 정서적 안정을 도모하는 심리상담 프로그램 마련의 필요성 등 사회복지실천에의 함의를 제시하였다.
Abstract
This study, based on grounded theory, explores the experience of a struggle against breast cancer of low-income single mother. Interviews were conducted with six single mother with breast cancer. With the permission of the participants, the interviews were recorded with the digital recorder and transcribed. Data were collected with in-depth interviews and observations until they were saturated. The interviews were analysed according to the constant comparative method used in grounded theory. The study derived 117 concepts, 30 subcategories and 13 categories in open coding. A core category was ‘passing through hash wind with responsibility on thin ice' in selective coding. And the process of struggle against breast cancer of single mother takes 'stage of diagnosis', 'stage of confusion and hardship', 'stage of coping' and 'stage of hope or despair'. The findings of the study will be useful in seeking an effective social welfare intervention plan for successful adaptation of low-income single mother with breast cancer.
