의사보조인력(Physician Assistant)의 현황과 의료법적 문제점

의사보조, 임상등록간호사, 진료지원인력 등으로 불리는 의사보조인력(Physician Assistant, 이하 PA)은 환자를 돌보는 의사와 한팀을 이루어 진료의 원활함을 위해 발생된 것이다. 실제임상에서 면허증을 가지고 있는 보건의료종사자로 이 체계는 각 나라마다 다르게 시행되고 있다. 법적으로 인증이 되는 의사보조인력제도도 여전히 많은 논란이 되고 있다. PA제도는 미국에서 1960년도에 시도가 도입되었다. 이 연구는 다른 나라의 PA제도를 살펴보고 우리나라의 PA현황과 법적인 문제 등을 살펴보고자 하였다. 각 나라마다 이 제도의 시행하는 방식은 다르지만 의사보조인력은 반드시 의사의 감독이 있어야 하며 실제 임상치료를 위해 면허가 있어야 하며 법적인 보호를 받고 있다. 그러나 우리나라에서는 이 제도에 대하여 법적인 뒷받침이 없다. 미국, 영국, 캐나다에서는 의학교육을 바탕으로 하는 학위프로그램으로 의사보조인력을 위한 많은 교육이 존재한다. 아직 한국에서는 PA를 교육하기 위한 전문적인 교육과정이 없지만 실제임상에서 많은 보조 인력들이 의사보조의 역할을 담당하고 있어 늘 법적인 문제를 안고 있다. 현실적으로 PA문제를 해결하기위해서는 법적인 뒷받침이 있어야 하며 실제를 행할 수 있는 범위를 정하여 보다 현실적인 PA제도가 만들어져야 할 것이다.
Abstract
Physical assistant, such as physician associate(PA), advanced practice registered nurse, is defined a kind of healthcare professional who is licensed to practice medicine as part of a team with physicians, and system of PA is different in each country. Although PA is being carried out legally in the country, there still is a lot of controversy. The PA profession was first proposed in the United States in the 1960s. This study aim to search to PA systems in other countries and investigate whether there is a problem in PA in Korea. Although implementation of the system is different in each country, the PA must have a primary relationship on file with a collaborative physician supervisor under depending on the specific laws and PAs have their own medical licenses with distinct of medical practice in US. But there has still not been legally recognized in Korea. There are many educational programs which are graduate programs based on medical education in US, UK, and Canada. In Korea, there is no training program for PAs but many clinical assistants are in charge of the PA role and this is always facing a legal issue. In order to solve this problem, there has to discuss each group such as doctor, nurse, and health government and establishment for the legalization of the PA system and an exact description of their duties.
