한국과 미국의 의료보험개혁의 비교 및 시사점- 문재인케어와 트럼프케어 비교를 중심으로 -

초록
외국사람들이 우리나라를 방문했을 때 우수하다고 손 꼽아 칭찬하는 서비스 중의 하나가 의료서비스이다. 한국 사람들은 우수한 품질의 의료서비스를 매우 신속하게 저렴한 비용으로 누리고 있다는 것이다. 우리나라는 혜안을 가진 정치가와 의료인에 의해 건강보험제도를 일찍이 도입하였고, 그 덕분에 우리는 지금 값싸고 품질 좋은 의료서비스를 누리고 있는 것이다. 그러나 100% 완전한 제도는 없기에, 건강보험 보장성을 좀더 확대하고자 하는 방향으로 문재인케어가 도입되었다. 그런데 건강보험 보장성을 확대하여 국민의 의료비부담을 줄이겠다는 선한 의도로 시작된 문재인케어가, 곧 어려움에 처하게 된다. 경영수지악화로 인한 의료계의 반발을 야기하였고 비급여를 줄이겠다는 목표는 달성하지 못한채 의료비만 계속 증가하고 있어서 보장성을 확대하겠다는 목표달성에 실패하고 있다. 반면에 의료비가 비싸 사회적 문제가 심각한 미국에서는 우리나라의 우수한 의료보험제도를 벤칭마킹하여 강제적 의료보험가입제도를 핵심으로 한 오바마케어를 도입해 의료보험개혁을 시행하였다. 그런데 오바마케어 시행이후 의료보험혜택을 누리지 못했던 저소득층의 환영을 받았으나, 보험료 인상에 중산층은 불만을 품게 된다. 트럼프는 바로 이러한 점을 파고들어 주요 선거공약으로 오바마케어를 폐지하고 새로운 의료보험제도를 도입할 것을 내세웠다. 그러나 예상 외로 트럼프의 의료보험개혁은 의회를 통과하지 못하고 실패로 돌아가게 되었다. 본 논문에서는 문재인케어와 트럼프케어를 비교해보고 각각의 문제점과 함께 찬반의견을 살펴보고 시사점을 도출하였다. 비교 및 시사점을 도출하는데 있어서, 품질, 접근성, 비용 세가지 관점을 중심으로 검토하였다.
Abstract
Korean medical service is regarded as one of the best services in Korea by many foreigners. Koreans enjoy speedy and excellent medical service with reasonable price. A great statesman and healthcare providers introduce mandatory medical insurance system. Therefore Koreans are benefited from those pioneers. However, there is no perfect system. Moonjaein care was introduced to extend the medical insurance coverage. Even though Moonjaein care has initiated with good intent, it met with strong objections. Healthcare providers object strongly due to worsening of the hospital management account and Moonjaein care seems to be failure due to increasing medical expenditure and standing ratio of non-payment of medical insurance. The U.S modeled after a successful Korean case and introduced Obamacare featuring mandatory medical insurance. The lower class welcomed Obamacare because they can have affordable medical care insurance. However, the middle class dissatisfied with increased medical insurance fees. Trump presented abolishing Obamacare and introducing new Trump care as election pledge. However, Trump care did not overcome the hurdle of congress unexpectedly. In this article, comparative research between Moonjaein care and Trump care will be done and the implication will be suggested by the perspective of quality, access and cost.
