초록
2020년 1월 20일 국내에서 COVID-19 첫 번째 확진자가 발생한 이후 같은 해 12월 12일 기준 처음으로 일일 확진자가 1천 명을 넘어서는 등 2021년 1월초까지 3차 팬데믹이 지속되고 있다. 향후 포스트 코로나 시대를 대비하여 감염병에 보다 효과적으로 대응하기 위해서는 신속하고 정확하게 감염원과 감염경로를 찾아낼 수 있는 방안을 모색해야 한다. 본 연구에서는 현재 감염병예방법에 따른 허용 범위 내에서 COVID-19 역학조사 데이터 분석에 주로 활용되고 있는 ①휴대전화 교신정보, ②신용카드・교통카드 사용명세, ③의료기관 및 약국 진료 및 처방전 내역(DUR)을 결합한 맞춤형 분석 방법론을 소개하고, 포스트 코로나 시대를 대비한 정책적 대안을 제시하였다. 먼저 미확인 감염경로 발견을 위해서 사회연결망분석과 공간분석 원리를 적용하여 1) 확진자 전파 네트워크, 2) 감염경로 추적, 3) 감염장소 추적을 위한 분석 모델을 제안하였다. 아울러 이러한 분석모델이 어떻게 역학조사에 적용될 수 있는지 해양수산부 집단감염 사건의 감염경로 추적 시나리오를 통해 명확히 하였다. 또한 역학조사관이 확진자 등을 만나 인터뷰한 자료인 역학조사서 정보를 추가로 활용하면 보다 정확한 감염경로 재구성이 가능함을 확인하였다. 정책적 측면에서 이러한 분석은 전염의 집락들 간 다리 역할을 하는 연결망 상의 ‘웜홀’을 발견하는데 유용하며 생활 방역 정책에 응용될 수 있다. 즉, 1) 지리적으로 긴 거리를 연결하는 웜홀이 통과하는 장소적 허브를 통제하거나, 2) 사회적으로 긴 거리를 연결하는 웜홀을 최소화하기 위해서 사회적 거리두기 방식을 기존의 접촉 빈도를 줄이는 방향에서 사회 활동의 성격, 즉 접촉 성격을 단순화하는 방안으로 유도할 수 있다. 연구의 측면에서는 분석의 초점을 사회적 허브가 초기 확산에 미치는 역할에서 길고 강한 연결이 재확산에 미치는 역할로 전환할 것을 제안한다. 감염병을 확산시키는 사회적 허브가 사전 식별이 어렵다는 점을 고려할 때, 이러한 초점의 전환은 허브 중심의 산발적 소규모 집단감염이 집단 간 확산되는 것을 보다 효과적으로 막을 수 있을 것이다.
Abstract
Since the first confirmed case of COVID-19 in Korea on January 20, 2020, daily new cases exceeded 1,000 for the first time on December 12, 2020 and the third pandemic continues as of January 2021. In order to respond more effectively to infectious diseases in preparation for the post-corona era in the future, it is necessary to find a way to quickly and accurately find the source of infection and the path of infection. In this study, we introduce novel investigation methods by integrating three sources of information that are allowed by Korean Infectious Disease Control Act: ①mobile-phone location information based on cell tower, ②credit card and transportation card payment information, and ③treatment and prescription history at medical institution and pharmacy (DUR). We propose specific methods to improve 1) constructing transmission networks, 2) tracking infection routes, and 3) locating infection sites. We also illustrate how these methods are applicable to epidemiological investigation, based on abduction and retroduction, by providing scenarios for the case of the mass infection at the Ministry of Oceans and Fisheries. In addition, we suggest that proposed methods could be complemented by epidemiological interviews with confirmed persons. In terms of policy, our study is useful in discovering a ‘wormhole’ in a global infection network that bridges between infection clusters, and can be applied to everyday life quarantine policies in two ways: 1) controlling locational hubs through which geographic wormholes pass to connect long-distance clusters, and 2) suppressing cross-cutting social interactions between different social clusters, rather than suppressing frequencies of social interactions, for the sake of social distancing. In terms of research, our study proposes to shift the focus of analysis from the role of social hubs in the early stage of infection to the role of long and strong ties in later stages. Considering that high-risk social hubs are difficult to detect in advance, this shift of focus may prevent the spread of virus between local clusters more effectively.
