원가패널기관 참여가 의료기관의 이익 개선에 미치는 영향

본 연구는 2017년부터 2020년까지 한국보건산업진흥원에 재무제표를 공시한 전체 종합병원들을 연구대상으로, 원가패널기관 참여가 의료기관의 수익성 개선에 미치는 영향을 검증하였다. 본 연구의 구체적인 분석 결과는 다음과 같다. 첫째, 원가패널기관 참여 전후의 이익 개선 효과를검증한 결과, 원가패널기관 참여는 당기 의료이익을 개선한 것으로 나타났으나, 당기순이익과 차기 이익에는 모두 영향을 주지 않았다. 둘째, 원가패널기관 참여 전후의 이익 개선 효과가 업무협약 패널기관과 신포괄수가제 참여기관이 다른지 검증한 결과, 업무협약 패널기관의 순이익 개선 효과가 신포괄수가제 참여기관보다 더 큰 것으로 나타났다. 셋째, 원가패널기관 참여 전후의 이익 개선 효과가 공공의료기관과 민간의료기관이 다른지검증한 결과, 공공의료기관의 순이익 개선 효과가 민간의료기관보다 더 큰 것으로 나타났다. 넷째, 원가패널기관 참여 전후의 이익 개선 효과가 도시규모에 따라 다른지 검증한 결과, 소도시 의료기관의 순이익 개선 효과가 대도시 의료기관보다 더 큰 것으로 나타났다. 다섯째, 원가패널기관 참여전후의 이익 개선 효과가 병상 규모에 따라 다른지 검증한 결과, 400병상 미만 의료기관의 의료이익과 순이익의 개선 효과가 400병상 이상보다 큰것으로 나타났다. 본 연구의 의의는 원가패널기관 참여가 원가관리를 통해 의료기관의 수익성 개선에 긍정적 영향을 미친다는 것을 보여줬고, 이러한 수익성 개선 효과가 업무협약 패널기관 여부, 설립유형, 도시 규모와 병상 규모의 차이에 따라 다름을 입증한 것이다.
Abstract
The present study, a research studied starting 2017 until 2020 regarding all general hospitals, validates the profit improvement effect that the cost panel agency participation has on the profitability. First, it showed that the cost panel agency participation improved this year’s operating profit. However, shows no relation to both the net income and next year’s profit. Second, it showed that net income improvement effect was bigger in Business Agreement Panel organized hospitals than RDRG(Refined Diagnosis Related Group) Participation Hospital Agency. Third, it showed that net income improvement effect was bigger in public health institutions than private health institutions. Fourth, it showed that net income improvement effect was bigger in small towns’ medical institutions than medical institutions of metropolitan areas. Fifth, it showed that the operating profit and net income improvement effect was bigger in medical institutions with under 400 beds than ones of 400 beds and up. The current study’s significance showed that the cost panel agency participation has a positive impact on medical institutions’ profitability growth through cost management, as well as authenticating that the such profitability growth effect differs depending on the presence or absence of a business agreement panel organized hospital, publicity, city size and the number of beds.
