과거질병이력과 건강검진지표에 기반한 치매 발병 예측모형 개발 - 국민건강보험공단 노인코호트DB 사용

본 연구는 건강보험공단의 노인코호트DB를 사용하여 개인의 건강검진 데이터와 과거질병이력 데이터의 치매상병코드를 이용해 통계적 예측모형을 개발하여 우리나라 노인들의 객관적인 치매 발병 요인을 분석하였다. 노인 치매 발병에 가장 큰 요인인 연령을 제외하고 치매 발병에 영향을 주로 미치는 요인은 최근 5년 동안의 질병 발생 여부이다. 최근 5년 동안 뇌질환, 파킨슨병 유병 여부, 7일 이상 계속 치료, 입원, 당뇨병, 골절 유병, 허혈성심장질환 유병, 수술, 고혈압 등이 있을수록 노인 치매 발병률이 높아지는 경향을 보였다. 다음으로 건강검진 지표들이 노인 치매 발병에 영향을 주는 중요한 요인이었다. 감마지티피, 혈색소, 공복혈당, 총콜레스테롤, 수축기혈압, 이완기혈압 등 건강 상태가 치매 발병에 영향을 주는 중요한 지표임을 함께 보였다. 본 연구에서 제시된 치매 발병률 추정값은 치매 리스크에 노출될 수 있는 미래 치매 환자들과 그 가족들의 경제적 리스크를 준비할 수 있는 보험상품 개발에 활용되고 개인별 과거 병력 및 건강 상태에 따라 위험을 세분화할 수 있을 것이다. 또한 노인코호트DB를 사용하여 실제 노인들의 과거 질병 이력과 건강검진 지표들을 통해 치매발병 상대위험도 예측 모델의 개발을 통해 치매 예방 연구에 새로운 방향을 제시하고자 한다.
Abstract
This study aimed to identify the factors contributing to dementia onset among the elderly in South Korea by utilizing individual health check-up data and past medical history data from the National Health Insurance Service's Elderly Cohort Database. Additionally, it sought to develop a statistical prediction model to estimate the objective incidence rate of dementia. The analysis revealed that age is the most significant factor influencing dementia onset, with the likelihood of developing dementia increasing considerably as age rises. The second major factor affecting dementia onset is the occurrence of diseases in the past five years. Higher incidence of conditions such as brain diseases, Parkinson's disease, hospitalizations and diabetes was associated with a higher rate of dementia among the elderly. Health check-up indicators were also presented as significant factors influencing dementia onset. The estimated dementia incidence rates presented in this study could be utilized in developing insurance products to prepare future dementia patients and their families for the economic risks associated with dementia.
