비정규직 활용이 환자안전에 미치는 영향: 미국 요양원의 질 평가지표 분석을 중심으로
The Impact of Nonstandard Work Arrangements on Patient Safety: An Analysis of Quality Indicators in U.S. Nursing Homes

여러 사회적 참사를 겪으며 안전사회에 대한 요구가 높아진 가운데 비정규직 고용이 초래할 수 있는 잠재적 위험에 대한 우려가 크다. 비정규직 인력의 활용은 업무 전반의 숙련도 저하, 책임성 약화, 업무 연속성 및 팀워크의 훼손을 초래할 수 있으며, 이는 생명을 다루는 보건의료 분야에서 환자 안전과 의료 서비스의 질에 심각한 위협이 될 수 있다. 그러나 보건의료분야에서 비정규직 활용이 가져올 수 있는 위험성은 주로 일화적 증거에 기반하고 있으며 이를 실증적으로 뒷받침하는 양적 연구는 부족하다. 이에 본 연구는 미국 뉴욕주 요양원(nursing homes)을 대상으로 비정규직 인력 비율이 환자안전에 미치는 영향을 실증적으로 분석하였다. 뉴욕주 보건국, CMS, 브라운대학이 공시한 요양원 평가자료와 운영자료 등을 결합하여 2018년, 2019년, 2021년으로 구성된 패널 자료를 구축하였으며, 비정규직 간호 인력의 비율과 보건감독 평가등급(Health Inspection Rating), 그리고 잠재적으로 예방가능한 입원(Potentially Avoidable Hospitalization) 간의 관계를 분석하였다. 고정효과모형 분석 결과, 비정규직 비율이 높을수록 예방가능한 입원비율은 유의하게 증가하였으며, 보건감독 평가등급은 10% 유의수준에서 낮아지는 것으로 나타났다. 이는 비정규 고용형태가 환자안전에 부정적인 영향을 미칠 수 있음을 보여주는 양적 결과로 비정규직 정규직화 정책의 실증적 근거를 제공한다는 점에서 의의가 있다.
Abstract
As public demand for a “safe society” grows, awareness of the potential risks associated with contingent labor is also increasing. Nonstandard work arrangements are widely used even in the healthcare sector—where human lives are directly at stake. The use of a temporary workforce can lead to diminished skill levels, reduced accountability, weakened teamwork, and communication challenges, all of which pose threats to patient safety. Despite growing public concern over the risks of such practices in healthcare settings, quantitative evidence remains limited. To address this gap, this study investigates the impact of the proportion of contract/agency nursing staff on patient safety in New York State nursing homes. A panel dataset was constructed for the years 2018, 2019, and 2021 by combining publicly available data from the New York State Department of Health and the Centers for Medicare & Medicaid Services. The study examines the relationship between the use of contract/agency nursing staff, health inspection ratings, and potentially avoidable hospitalizations (PAHs). Fixed-effects model results show that a higher proportion of contract/agency staff is marginally associated with lower health inspection ratings and significantly associated with increased PAHs. These findings suggest that nonstandard work arrangements can have substantial impacts on patient safety and underscore employment stability as a key determinant of care quality in long-term care setting
