This study aims to propose a Sports ESG management model as a sustainable survival strategy for provincial cities facing the risk of population extinction. While traditional urban development paradigms have focused on housing supply and physical infrastructure, this research suggests a necessary transition toward vitality-driven cities centered on sports diversity, aligned with the current era of demographic decline where housing saturation has been reached. Empirical data analysis of high-risk extinction zones confirms the health-economic value of sports participation, specifically in reducing elderly depression and lowering annual per capita medical expenses. To overcome the low fiscal self-reliance of local governments, this study derives a Corporate-led Public-Private Partnership (PPP) model, where private enterprises maintain management rights for a set period before returning facilities to society. The findings verify that diversity in sports disciplines serves as a retentive engine for residency, effectively substituting for conventional housing welfare. Furthermore, the study validates the feasibility of governance innovation and operational cost reduction through private management techniques and analyzes the economic impact of "Daily Legacy" created by establishing off-season training hubs and hosting frequent, small-scale events. In conclusion, local sports ESG management must transcend simple social welfare to become an industrial engine for regional survival, necessitating a shift in policy-based budgetary structures and the establishment of formal legal guidelines.
